• Send an e-mail to sales.info@capellatech.com including a sample of what you would like the form to look like (i.e., text/graphics positioning, size of form, logos, etc.)
• An E-Forms specialist will contact you to discuss the details of your form requirements
• Once the form and design has been defined, we will e-mail or fax you a draft of your new custom form within 5 business days for approval

• We will email you our digitizing forms and questionnaire to define your format needs
